Wouldn't it be more of a punishment for the doctor to live with it? Have nightmares about it? Have it on his conscience every day?

That's kind of a huge assumption about how moral the doctor even is, considering what we know about him.

Yeah, and doctors are typically desensitized to matters concerning health. In fact, they're encouraged to view death and failure objectively, and impersonally. It's not surprising that a doctor, or even an entire hospital would shamelessly cause serious harm to a child or family to cover up an isolated case of malpractice (which is basically the death knell of any private hospital if it gets out). Assuming that people will feel bad about things they did selfishly is naive. People grow numb to guilt by the time they've become a political force of any degree, generally.

I've heard from people in the health care industries that scrubs is pretty realistic about how people come in bright eyed and bushy tailed and end up jaded.

I can also see how this might have been one bad decision snowballed with everyone doing what they thought was best. The cops, the parents, social services, the other doctor, everyone drawn into the crossfire by doing what they thought was best.
