I think in this case, transformation is referring to more of a sentai type of thing, than any kind of severe body modification.

Madam would probably mean mistress since Femdom has it's own tag but usually that's just tagged as BDSM so I have no idea what it could really mean. But the fact that there's insult supports the mistress thing.
SF...only thing that comes to mind is Science Fiction and that seriously doesn't look like it applies.
Transgender is tagged as TF so I doubt Transformation Heroine means that, it's must likely sentai-ish like previously mentioned.

I'm not sure about this one so I'll wait to hear some impressions.

"Madam" is a bad translation (since it suggests age). The original is "ojou-sama", or "young lady". Yannow, the stuck-up rich girl with a 'refined' manner of speaking you see in some anime.

As a side note, the transformation tag is apparently there because the protagonist transforms in a sailor-moon esque way to fight.

I can only find 3 worms in the first city. Where are the other 2? There is one in the building in the first area and one in a building in the 2nd area and one walking around in the 2nd area.

EDIT: Nvm found them walking around...
