Ok, since I've been getting PMs this is the closest thing I can do to make the guide as clear as possible. I just played through it while writing it to confirm all the information so this /should/ be 100% correct at this point - aside from potential unnecessary steps.
-Because I'm unable to confirm it otherwise, assume you have to be on the second playthrough (New Game+) to do this. However you might not need to be, this is just the way *I* did it-
Forest:
Area 1 - Aluraune, no choices
Area 2 - Nekomata, no choices. Event at the end with Seimei (Dark skinned woman)
Area 3 - Fairy, unwinnable fight with Fang-Mei (Kyuubi) at the end. Choice - Pick Top
Area 4 - Giant, "!" space leads to conversation with Seimei. Choice - Bottom. During H-Scene: Choice - Bottom.
Area 5 - Slime Girl, Octopus Girl, Fang-Mei. When fighting Fang-Mei if you're unable to do enough damage to her before the game decides you've "lost" (Not the same as losing your health) then you'll be forced to make a single choice. If you did the above choices and did enough damage to Fang-Mei within the time-limit, then you'll have two choices. Choice - Bottom.
After beating Fang-Mei there will be some dialogue. Choice - Top.
After more dialogue you'll be in a fight with Orochi, just like Fang-Mei before this is un-winnable.
Castle:
Area 1 - Cyclops, once you pass through the talisman wall you'll have an event with Seimei. Choice - Bottom.
Area 2 - Naga/Lamia (w/e), once you pass through the talisman wall you'll run into Fang-Mei and her Slime servant. Choice - Bottom.
Area 3 - Kamatachi, no choices.
Area 4 - Yuki-Onna and Worm Princess. There's a choice in the middle route of the level that will put you on the route you want. Top choice is the Empress/Orochi route. Middle choice is Fang-Mei's route. Bottom choice is Seimei's route. This influences what final boss you will fight (And the H-Scene you get upon winning or losing).
Area 5 - Spider girl + Variable bosses.
If you're on the Empress' route you'll have to fight Fang-Mei and Seimei as back to back bosses before the Empress' chamber, once you enter you'll have another choice. First choice weakens the final boss by 500 HP, but there's no H-Scene upon victory. Bottom choice has the Empress at 1500 HP, but you are awarded with an H-Scene upon winning (Ala VH2 or VH 1).
If you're on Seimei or Fang-Mei's route the event that would normally have their fights is replaced with a scene of the empress being defeated by them. They will replace her as the final boss in their respective routes.
Losing to any of the final bosses in their second 'form' (Post music change) results in a game over H-Scene. Losing to them before this results in game over, like Xueli in VH2.
After beating all the routes and attaining 100% CG gallery the extra tab will open up on the main menu beneath the game exit button. Enjoy the threesome scene.

I feel compelled to type my thoughts out on this game.
I'm not a massive fan of these games for the fact that I'm not a big fan of shota on monster girl. Beggars can't be choosers though, and a beggar shouldn't just say no to something of this quality just because it isn't exactly what he wants.
Which is the thing, this game was of high quality, just like the last two. As far as the hentai goes, anyway. Which is objectively the most important thing about these games. Things were drawn nice, the girls were cute, and there was a fair amount of content. This is enough to give the game a thumbs up.
Everything else was tedious. More so than the last game. The labyrinths you crawled around prior were more interesting to move through and I felt had more substance to them. In this game it feels like a monotonous game of Mario Party, going across a very linear board that you have to restart every time you reach the end of a narrow path, and then go down another. Only to be broken up by the random encounters you'll face before boss fights.
Personally I think if they had worked the environments more to work like a game board, it'd of been more fun. They were KIND of going for that anyway, so why not?

I expected this to be an RPGMaker Game.... it lagged a lot, it had strange gameplay mechanics I didn't understand, and the window stretched further than the screen even allowed for me. I passed this on. I hope someone else enjoys this.

Anyways, the game itself is ok. The h-scenes and artwork are great, like the previous 2 games. My issue with the game is that it's not a consistent grind because of the one-way board game maps. If you guys played VH2, you should remember grinding out potions in the long hallway with the 4 bosses (or at least, I had to). That process was smooth since it was consistent. This game, you're usually underleveled when you hit a new area, so you want to grind a little and open up the map before the bosses. The issue is because the map is one way, it's not a consistent grind. You basically have to run through a path, gaining maybe 1/4 of a level, go back to the world map, and repeat until you hit the a good level. The game provides a ton of items though (I usually get an item drop after each enemy I beat), so you should never be strapped on items.
All in all, it's a good game that has it's flaws.
Dieselmine games are all based on the Kirikiri engine (at least all the ones I've played).

Move your cursor to the top of the screen when in a dialogue event or at the level select menu.

It's actually rather fast paced if you know what you are doing and save after each area in case one of the game overs forces you back to the title screen. (Slime maid enemy in the forest did that). I blew through the forest pretty quickly.

Losing to the slime maid enemy is considered an 'ending' of sorts, that's why. Just like losing to the worm princess in the castle also sends you back to the title.
