If he decides not to add certain enemies or things, maybe he just wasn't all that hyped about the idea of them in the first place. I don't think there's anything wrong about being swayed by your audience sometimes. It just means that you would rather please certain people more than add a certain thing, or that you weren't all that interested in adding it anyways. Not like anybody is able to force him to add or remove anything, it's all up to him in the end. It's easy to say "I'll just do whatever I want" when you don't have fans telling you that they're losing interest in your game over something "relatively" small.
By that I mean, compare the amount of people who will avoid his game because of a certain element (Unavoidable heavy guro and scat) versus the amount of people who would only play his game if they contain them, and I'm almost 100% positive you'd be losing a lot of fans by adding them. I doubt there are many people who will be like: "NO SCAT?! I'M NOT PLAYING THIS GARBAGE!", but the idea of that does make me laugh. Eluku's games are already pretty borderline guro, so I also can't imagine the kind of person that would avoid it just because it's missing some torture either.
I personally don't like either, but if he really wanted, there's always the option of making it toggleable, unless that's not possible for some reason with the system he's using. Then everybody could be happy.
