The goal of such media is to instill either a sense of dominance or submission depending on which character you project yourself as (hope I'm not going too far into unfamiliar territory here)
Another prime example of how outrageous and downright excruciating torture can be sexual would be the fairy fighting game, where the character evidently gets screwed by objects larger than her limbs, gets eggs planted inside her breasts and god knows what else, the only difference being that there is no visible blood to indicate permanent physical damage
