Yes indeed. You can also get a decent orange armor in first town, it seems this armor can't be damaged or takes too long to break. The one with armor on breasts.

If I'm not mistaken you have to find Salamandra blood or something like that, I think it's in the tower somewhere.

Certain enemies have cloth-breaking attacks, anything else doesn't do anything. (near the start, I believe the bees are one of them, the worm in tower definitely is one)
.- . Also, I believe the heavy armored clothing (the one that can't be used for the bar event) is immune or highly resistant to cloth breaking attack.
Ah, no, sorry. What I meant was if there is a penalty for getting hit with those attacks. In Magika it was your sanity meter, etc.

Blacksmith asks for a (permanently) enchanted sword to examine. From demo - I haven't played full game yet. Believe that he examines it and returns.

The karma level is on the right side on the menu when you open it up i believe) some % (left side % is satisfaction I think). It seems to go down randomly... and or possibly bugged. At one point I got to 6%, but it dropped to 3% and hasn't moved for awhile (even for events that say karma raised)...

Would anyone mind compiling a quick list of places where you can get H-scenes? They seem rather elusive.
