A heavily inspired by Demon's Soul game.

It has some interesting mechanics in the H content (each zone target specific area, breast/ass/vagina/clit and thus the fetish on them differs) in that you can keep the modified body into the different zone so you can have some interesting combination going.
(based on the decrypted file, here's breast expansion, clit->penis, etc, sadly they don't seem to be available in the demo)

(no H content from the initial orc/1st boss)

Much like how you probably get to meet Mr.Cowpad (Phalanx) the first time in Demon's Soul.

Go left, keep going, hit a lever, keep going back to the castle entrance, go in, boss fight.

And yes you do need to kill the first boss before meeting any H content.

You don't need to go back - there're stairs. But I find this game quite strange. First of all, behaviour of Ctrl is diturbing me, then some strange stats at the menu screen and strange pink gauge at the right-top.
H-system is also strange - after being defeated by some boss you're thrown in a cage with monsters with H-attacks and only 1 animation per cage (not depending on monster).
Strange.

A fair bit of it, actually. Other than that, pretty standard turn based combat though.

Ctrl double the game speed, it can be toggled on/off (one of the yes/no question at the start tells you about it)

Most of the stats on the menu are related to H content (usually 'is there parasite at X'), the pink bar is orgasm meter.

As for being thrown into monster den that applies to any zone (besides the starting area/boss), it's meant to be for racking up H attack count on that zone (since you're facing 4 mobs + they have high chance of doing H attack) by just doing the 'give up' to let them beat on you (not suicide skill, they're 2 different ones)
The decrypted files show things like breast enlargement, probably as a result of large amount of breast sucking (doesn't seem available in the trial though)

To get out just pick 'yes' after you die to any monster in the den.

Body mods that actually persist after the scene and have an impact in the rest of the game are pretty much the best thing ever.

Hope this doesn't disappoint.
