How much of the focus do you want to be on the preg content?
Evil Goddess by Newface has pregnancy status that causes belly inflation and I think possibly some status debuffs as well.
I remember each character could unlock equippable titles for H-related achievements, and the pregnancy buffs were really some really massive tankyness boost that was great for the early game. I remember trying to grind 100 pregnancies on all my characters so that I could focus on offensive stats on their gear instead.

well ofc its not endings and preg content is the mechanics and had visual not just some debuff without picture (they had preg sex on it ofc)

1. Knights of Messiah
This game has so much of it that it's just plain silly:
There is a mechanic where enemies will take a potion that makes them one shot, on birth, and rape your companions mid battle. Or you can choose for them to whore themselves out, with a similar effect. So by game's end they may each have 20 plus kids, who you can go meet at the Church.
2. Milf Selena and the Dungeon of Magic stones. By Resentiment/Dieselmine available on Steam.
In this one, an evil guild tricks a retired adventurer into singing a contract that makes her their sex slave. She is forced to sleep with her allies after adventuring with them every day, and at various cutscenes as you progress, but nothing happens if she loses in battle. She can get pregnant and give birth quite a lot of times. It's really messed up, because her daughter she has with her husband gets involved at some point.
3. Princess of Zeven.
Originally called "Pregcess of Zeven" , it's about princess trying to conquer a dungeon. She can give into requests from other people in town, and get knocked up.
4. Violated Princess
This one I haven't played very much of, but I'm pretty sure she can get knocked up and keep going. Not absolutely certain.
