You don't "have" to download anything. Everything is optional. If you want to play without mods that's your choice.Do I have to download all mod packs? or just choose one. For example the last Modpack R4.9 that came out or do I need any extra add-ons?
What "all" modpacks? Nothing stops you from dumping everything you find on the interwebs in the Mod folder. But don't cry here when things stop working. As a rule of thumb you should never overlap large modpacks.
What "extra add-ons"? I mean besides the 2 additional packs I mentioned in the guide. One is for official translation and the other to have a proper plugin setup on top of CMI because most people just choose Basic preset in CMI and move on. Then they wonder why downloaded presets look like shit.

is the only add-on for the game that isn't optional. Unless off course you're fine with pixelated genitals and want to play the game that way. Then the other mandatory "add-on" would be a game update that is supported by latest CMI. The torrent in op is version 2.39.2 so you should be fine with that for the moment. 2.40.0 is currently under investigation.
If by add-on you refer to other DLC then you can install them manually from the list, if you want to. If you don't know what list then there's no point in having this conversation.
I suggest waiting for R4.10 at this point but if you want to give R4.9 a try to see what you're dealing with then that's on you.
Latest modpack release always includes everything from previous versions.
I rarely remove anything from it unless something is redundant, unfixable for my skill level or was updated by author with different file names and it is too much work to figure out the changes and a cleanup is better. Otherwise I maintain the file names as to not break older presets that used them.
