The whole area. Think of it this way, each biome has it's own theme. So, wherever the theme can still be heard still counts as the area. Any place where the music will change to signal a new biome would undo it. But now that I think about it, if were just music based, it could help in some situations. For example when facing the Mummy Girl, the theme changes when she traps you in the corridor. This could undo the god mode and allow a game over to take place so the player doesn't get trapped in there forever. This could also be applied the Red Hooded Ghost seeing you'll probably get stuck in a loop if she catches you while in god mode.
Same. Although if FOBS really had some game over cinematics for specific girls, I imagine hers would be along the lines of dragging the MC back into her hut after he's blacked out from exhaustion, only then to reawaken to a scene with him bound by a leash and collar while she proceeds to ride him.
