The thing to consider is the size of the market... Diablo 3 sold what? 6 million copies so far? An hentai game is lucky to get 1000 buys.
Even if you made the most amazing hentai-game ever and sold it for 1$, you'd be extremely lucky to break the 10000 sold copies.
Take specialised business software: Those usually costs you at least a few thousands dollars upfront with hundreds of dollars each year for maintenance. You could argue that there's less work involved in those software than there are in Diablo 3 (which would be correct), but their expected market numbers in the hundreds, not millions. It's the same for hentai games.
Like if you consider the "big" games: You have a whole heck of a lot of art to make. You got voice actors to pay, and the amount of voiced text can be immense for some titles. You got the programmers that need to put everything together. How can you expect to turn a profit if there's very few people that can buy your game? The hentai market is small to begin with and the audience is picky so it limits your potential sales further. For example, here's my personal list of requirements:
1) It's an actual game. I don't care for visual novels so it should be a game and have fun gameplay. Strategy games and platformers are my favorites.
2) Female main character: I will occasionally play a game with a male main character, but the game itself needs to be darn good for that to happen.
3) Absolutely no gore, no maiming or anything approaching that.
4) Have a possible happy ending. This means that the main character(s) are not mind-broken or slaves. They can face a whole heck of a lot of adversity, but there has to be the possibility to persevere through it and come out of it fine in the end.
5) You should be able to avoid getting your character(s) being raped if you play well enough. I'm willing to overlook a forced sequence if it makes sense (ala Raidy 2) as long as you at least get the occasion for some revenge later on.
So that limits what I'm willing to buy considerably (with some exception like this game here that I shouldn't have)... I'm sure that's the case for everyone else. You have a small market that you can only aim at a tiny subset of it...
* I'd LOVE seeing a game with good gameplay that has actual consentual scenes with tentacles or monsters (the heroine likes it because she finds it enjoyable, not because she's mind-broken)... But I'm really not holding my breath of ever seeing that. Sakyubasu no Tatakai is about as close as it gets I suppose
