Well the main thing is, it shouldn't be 'easy' to get. I'm thinking something like an end of route reward, similar to the parasite armor in the original game.

how about another cassie route, something like captured by the slaver ring when trying to investigate them and experimented for mega-stuff, so at first the player will have the mega-stuff with debuff, at the end of the route the player could choose to have the mega-stuff and debuff removed or just the debuff removed, probably the removal take place in new ark lab, and the lab could reapply the mega-stuff too, i mean in original lust doll the slaver tamper with cassie body, and linda have fantasy for mega-stuff too, soo i think it fits?

or maybe end of mystra 2nd/another route? since her original lust doll route gave player tentacle control ability, and in game over its shown that she could gave mega-stuff too

or something like a spell reward after completing another rinny route

or maybe could have 2 types, the easier one only for cosmetic and the harder one with buff(s), also if the player have the easy mode mega-stuff they can't get the hard mode mega-stuff, at least until the player "reset" the size
