Actually....
The DC for a concentration check is 10 + damage dealt. In the case of spell use, add the spell's level(3 in this case) to the DC. Being grappled while casting also has a DC of 20, but no other modifiers, though only spells w/o somatic components can be used.(But since the vampire can do the gaseous form at will, not sure.)
So, if a concentration check is necessary, the vampire must roll 3 concentration checks I believe. 1 for Holi's hit, 1 for Trex's hit, and 1 for Carella's grappling. DCs for for each in order would be 31, 30, and 20.
This would also have brought the DC for the concentration check I had to make up by 9, but I would have still passed by the skin of my teeth.
Or I could be completely wrong, but I don't think that's the case.
EDIT: Wouldn't Trex also get an extra attack from the haste spell?
Edit again: Tassadar, I think the vampire has to make a DC 25 Concentration check to use gaseous form
