Forgot to say but my affection at the time I posted was about 430 and my lust was at 175.

Apparently what I need to trigger the nurse outfit was to prepare drugs at least once.

He didn't remove them. Try going out alone at night, you should be able to go to the shop which sells the more provocative outfits and accessories.

While this is true, if you get Sylvie's love and lust values high enough she will run out of the house and into the street to greet you if you leave the house without her, regardless of what she was wearing before you left.

I dont think it is the lust or affection value but the amount of times that you sleep with her? My lust/affection is over 10k "edited" and yet nothing happened. Perhaps all 3?

Nope. It's only affected by affection and intimacy values (love and lust, in the code). While I looked at the code and technically 10k should work, perhaps you broke something with your nonsense. As I played the game normally, I haven't run into any issues with the going-out-alone events.

She has to be longing for you. If she's not actually begging you for sex, the event will not trigger. How high her lust is and how long you've made her wait determine which one you end up seeing.

Edit: I realised mousing over showed the unlock conditions, silly me. How do I unlock the 'apron' and the ability to work (presumably as a nurse?)
Edit2: It seems that you need to clear enough events to unlock further clothing (or perhaps its limited by intimacy). It seems the save files are easily modifiable so I'll experiment with those when I find the time.

Edit3: peeked at the translation; a few things you can try saying to sylvie:

Note that these simply trigger reactions from her if you ask her to call you those things. Just try asking her to call you something like that at lower love/lust levels and see what happens.

I haven't seen this happen to me yet
Been living with her ~140 days, intimacy 1000+ and lust 560.
Tried going out multiple times alone, and go straight home, as well as going into the restaurant first.
I've also done this while making her wait and deny sex twice, so her longing is high.

All I get is "the door swung open, she must have been waiting at the window".
