Well, text games can be fun if they have mid-combat rape.
Assuming you're just really bad at making a humorous yet constructive suggestion: this is a run or rape/survival game... Enemies basically beat or tease you to submission. If this is what you mean, the MC is basically always the sub, yeah (except for one minx and the still-humans).
If you mean BF, as in MC being able to sexually subdue the enemy... I wouldn't mind that at all, but it would be kind of hard to justify past the first few days. Not to mention the "omg how do I even f@ck that!?"-feeling anyone would eventually get at some point in the infection.

As you already know, feel free to ignore the Patreonist trolls. You know how they are: feeling entitled because they give away their lunch money to some con artists. Never mind them, they were born after freeware had died
I think the game is fine simple as it is. Just, unfinished. Honestly, MMM ceases to be an H-game and becomes a Resident Evil thing very early in the infection, as far as my vanilla tastes are concerned. I can't help much H-content-wise.
However, the Lovecraftian atmosphere is really cool, you can safely capitalize on that. The 'infection curve' is a strong mechanic of the game, I'm not sure how to make more use of it...
If anything, maybe give more life to the NPCs? CoC's forte was the companions system; escaping the island with a chosen waifu (for instance) would give some plot routes/twists. Since the playtime is limited, having different endings would add to the replayability.
Some cutscenes serve no other purpose than just ambience - maybe make them more interactive, with choices to make and consequences for the island? That would also be awkward with the 'helplessness' atmosphere...
Maybe make the initial 'MC personalities' have more consequences? A 'New game+' to avoid the grind on the first week? Or just unlockable starting bonuses, given you completed this or that event/challenge?
My advice would be, just do whatever you feel like doing... More cut-scenes, more side stories, whatever you have fun writing, really. Something like the gnomes/goblins/pixies? was entertaining; any strange idea you feel like implementing would probably fit in. Many mysteries remain unexplained, and the island has many(?) unexplored areas.
If you really are motivated, draw some different plotlines, each with 1+ endings. Mix in hidden stories, easter eggs, rare shiny monsters, funny silly stuff... And you can call MMM wrapped up.
